Student-Athletes and Parents,

 

            As the head athletic trainer, I consider it my privilege to provide athletic training services to you and your sons or daughters.  To allow me and my staff to provide the best athletic training services, we would ask for you to complete and sign the following forms and fax them back to the Erskine College Athletic Department by August 1st.  Your son/daughter will not be allowed to participate in any intercollegiate athletic activities until all paperwork is received and on file.  We would also ask for a copy of the front and back of your insurance card as well.  The fax number is located at the top of this letter.  All forms require Adobe Acrobat Reader in order to be downloaded.

                        3.         Assumption of Risk Waiver/Consent to Treatment

                        4.         Physician and Rehabilitation Services Policy

                        5.         HIPPA Authorization

                        6.         Emergency Contact Information

                        7.         Medical Release Form
                        8.        Concussion Information Form

                       

                       

            These forms are required of new student-athlete who have been diagnosed with ADD/ADHD.
                        1.    Letter from Dr. Leski, M.D.
                        2.    Medical Exception Form to be completed by family physician.
